Create a group practice record

To create a new group practice record, complete the following steps.

  1. Open the Group practices form.
  2. In Find Group, search for the group practice.
    • If the group practice is not returned in the search results, click the New.
  1. In the Group tab General section, complete the following steps.
    1. In Group, enter the name of the group practice.
    2. Optional. Type the group practice's Alternate Name, Group ID, Group Tax ID, and NPI information into the respective boxes.
    3. Optional. In From and To, type the respective dates of the group practice's existence.
    4. Optional. In Status, select the status of the group practice.
    5. Optional. If the group practice has been terminated, complete the following steps.
      1. In Terminated, type the date of the termination.
      2. In the Term. Reason, select the reason for the termination.
  2. Optional. In the Specialties section, click Add to select and add any specialties that apply to the group practice.
  3. Optional. In the Additional IDs section, click Add to add any additional IDs to be applied to the group practice.
  1. Optional. To add an address to the group practice record, complete the following substeps.

    1. On the Address tab, select one of the following options.

      • If no address records exist, click Create a new address.
      • If pre-existing address records exist, then near the top of the form click Add Group Address .
      Note

      The Address tab indicates which address you are currently viewing, such as Address 2 of 3. To view a different address, use the toolbar navigation buttons.

    2. To add another address to the group practice record, repeat the previous substep.

  2. Optional. In the Address Details section, complete any of the following steps with information specific to the address displayed.
    1. Select a Group Address Type from the list.
    2. Type the group practice ID, Tax ID #, NPI, and Alt. Name information.
    3. In From and To, type the respective dates of the group practice's existence at the specified location.
    4. In Home Page, type the website address that is specific to the group practice at the specified location.
    5. In Email, type the email address that is specific to the group practice at the specified location.
    6. In the Status list, select the status of the group practice at the specified location.
  3. Optional. In the Additional IDs section, click Add to add any additional IDs to be applied to the group practice at the specified location.
  4. Optional. In the Office Hours section, enter the office hours for this group practice for each day of the week at the specified location.
  5. Optional. In the Specialties section, click Add to select and add any specialties that apply to the group practice at the specified location.
  6. Optional. If your organization tracks additional information about a group practice address, in the User Defined section, enter the required information.
  7. Optional. In the Street Address section, type the address, related phone numbers, home page, and email address information into the respective boxes. (See Working with address validation)
    Warning!

    If any group practice or provider addresses have been attached to this group practice address record, editing the information displayed on this form will change this information in the associated records.

  8. Optional. To add providers to the group practice at the specified location, in the Providers section, complete the following substeps.

    1. In the Providers section, click Add .

    2. In Find Provider,find and Find (Select) Modal to be added to the group practice.

  9. Optional. In the Categories, Services, Languages section, click Add to select and add any categories, services, and languages that apply to this group practice location.
  10. Optional. In the Additional IDs section, click Add to select and add any IDs that apply to the group practice at this location.
  11. Optional. If your organization requires you to track additional information about the specific group practice address location that does not apply to any existing sections symplr Provider provides, in the User Defined & General Hours section, enter the required information.
  12. Click Save.
