Create a New Root Cause Analysis
The Root Cause Analysis (RCA) tool enables users to document specific information about an incident. Use the information below to create new RCAs.

From the main RCA tool page, click Create RCA.
- Enter the RCA Name in the required field.
- Enter a Problem statement in the required field. This field can include up to 1000 characters.
- Add one or more Participants to the RCA. Participants are team members for the RCA. These team members may not have permissions to create or view the RCA.
- Enter an optional Theme for the RCA. The theme can be used for sorting or reporting purposes.
- Enter the Incident date.The incident date can be a current or past date. Future dates cannot be entered.
- Select one or more optional, existing items in IAM to link to the RCA.
- Click Save.
Once the RCA is created, it is added to the RCA dashboard grid.

From the main page of the RCA, click the name of an RCA to open the RCA. Users with modify permissions can add causes to the RCA.

From the main screen for the selected RCA, click Add cause.

Enter a cause in the required field.

Choose a classification from the dropdown list. If no classification is selected, it will show as no classification.

Click Add cause.

A subcause can only be added to an existing cause. From the main page of the RCA, click the name of an RCA to open the RCA. Users with modify permissions can add subcauses to a RCA.

To add a subcause, select the Classification related to the new subcause and click Add subcause.
NoteIf a cause has multiple subcauses, scroll to the bottom of the grid to access Add subcause.

Enter a subcause in the required field. The subcause can be up to 1000 characters.

Click Add subcause.
The new subcause shows at the top of the grid. Repeat the steps until all subcauses have been added.
